to authorize another person to pick up your U-PASS at ID Center in Student Center East (SCE)

The ID Center and UIC are not responsible for any U-PASS not arriving at the intended destination by postal mail. U-PASSes are sent in standard envelopes with first class postage. No tracking information is available. If your U-PASS is lost after leaving the ID Center, you are responsible for a $50 replacement fee. * Required
